A worker was killed last week inside a Jacksonville highway jobsite when a concrete slab suddenly fell on top of him.

According to a report from First Coast News, 42-year-old Alexander Cordova was standing near a truck when a 7-1/2 ton slab of concrete fell from the back of the truck.

The site reports the accident occurred around 2 p.m. Friday. A crane removed the slab from the site around 6 p.m. Cordova was pronounced dead at the scene.

Florida Department of Transportation officials told the site Cordova had been hauling sound barriers to the site where work is being done to widen lanes on Interstate 295.

It remains unclear why the slab fell from the truck. The Occupational Safety and Health Administration is investigating.
